This page appears when you save the order from the Order Samples: Sample Information page and the order contains one or more order choices with a related clinical information question. If the clinical questions apply to a split order, then Harvest Webstation adds split order information to the page, such as "(order 1 of 2 split orders)."

1. Record the patient's answers to the given questions. If there is a default answer for the question, it will be displayed when the page appears.
• If a question has possible answers in a drop-down list, the "No answer given" option is selected by default.
• If a question is a multiple choice question, the answers are shown in a scrolling list box. Select the answer(s), using the Ctrl or Shift keys to select multiple answers.
• If a question is marked "[Required]," then you must answer it before you are allowed to save the order.

Note that you may modify your advanced configuration options to skip clinical information questions in Harvest Webstation, or to make "[Required]" questions optional instead.

2. If you are attaching the new order to an existing order, Harvest Webstation will display all questions that apply to the existing order and to the new order choices, one at a time, including the questions that were already asked for the existing order. You must answer the remaining unanswered questions (if any) before saving.

   IMPORTANT: Harvest Webstation displays only clinical questions for order choices you have the security settings to see. However, since one question can be associated with more than one order choice on the order, Harvest Webstation only omits a clinical question if all of its associated order choices are restricted (i.e., all tests in all order choices are restricted).

   If no clinical questions can be displayed due to security restrictions, Harvest Webstation displays a message that says "All clinical questions for this order are restricted."

3. Click the Save button to record the patient's answers and continue to the next clinical question or to the Unreleased Order Details page, which appears after you have finished answering questions.
4. The Unreleased Order Details page appears, displaying the order and confirming that the order was saved. From this page, you may view all of the order details, print the order, print labels for the order, print invoices, or delete the order.
5. The answers to the clinical questions will be included in the requisition for the testing location (if applicable) and transmissions to host systems (if applicable).
6. Once the order has been released into Harvest, you may view the answers to clinical information questions from the Orders in Progress page. See Orders in Progress.
