Accessing the Guarantor Page

Click the ... button on the Order Samples: Sample Information, Add Patient, or Edit Patient pages. See Order Samples: Sample Information or Edit Patient: Patient Tab.

General Information

  • A guarantor is the person responsible for covering whatever costs insurance will not cover. You can enter the information manually for individual orders or patients.
  • The guarantor may be the patient, or any of the relationships defined by HL7.
  • The guarantor information you add using the Order Samples: Sample Information page is considered order-level, and the guarantor information you add using the Add Patient or Edit Patient pages is considered patient-level.
  • Harvest Webstation displays the guarantor information, if available, on invoices and standard or custom requisitions. If you are using the custom requisition, then you will need to include the necessary field codes to make the guarantor information appear. See the Harvest online help. Note that the system administrator for Harvest may modify the advanced configuration options to omit the guarantor information on invoices, standard requisitions, and custom requisitions. When enabled, the Guarantor box will still remain on the form but will be empty.
  • When creating a new order that does not have order-specific guarantor information, but does have patient-specific guarantor information, Harvest Webstation will use the patient-specific information. However, once the order is created, even if you change the guarantor information for that particular patient, the guarantor status of the order will be unaffected.
  • Harvest Webstation does not store the patient-specific guarantor information with the order unless you open the Guarantor page and click the Save button. This allows the order to get the latest patient guarantor information when it is released into Harvest.

Entering Guarantor Information

  1. Enter or edit demographic information in the fields described below to create or modify the guarantor's file.
    • Last Name Enter the last name of the guarantor.
    • First Name Enter the first name of the guarantor.
    • Middle Name Enter the middle name of the guarantor, if applicable.
    • Address 1 / Address 2 / Zip Code Enter the address information for the patient. Zip codes are automatically linked to the appropriate City and State as they are entered into the system. As Harvest Webstation encounters a new zip code, it will prompt you to input the City and State, permanently storing that information in the system.
    • Phone 1 / Phone 2 Enter the telephone contact information for the patient.
    • Sex* Select the sex of the guarantor from the drop down list. When creating a new patient record, "< none selected >" is selected by default. This field is required because it is used to determine the proper reference range to report.
    • Date of Birth Enter the month, day, and year for the birth date of the patient.
    • SSN (Social Security Number) Enter the patient's social security number.
  2. Click the Copy from Patient button to copy demographic information from the patient over to the guarantor. Use this button to copy the last name, phone 1, phone 2, address 1, address 2, and zip code values. Note that Harvest Webstation will not overwrite any existing guarantor information. For example, if the guarantor already has a last name value, Harvest Webstation will not copy over the patient’s last name, but will copy the other information.
  3. When you access the Guarantor page from Order Samples: Sample Information, an additional button called Save as Patient Guarantor appears. Click this button to save the guarantor information you have entered to the patient record. See Edit Patient: Patient Tab.
    • This button only appears if you are editing order-level guarantor information.
    • If you do not have the security settings to view Social Security Numbers, then Harvest Webstation will leave the SSN field blank when you select to update the patient guarantor with the current guarantor information, because it may not be accurate to leave the existing SSN in place (for example, guarantor changes from mother to father). Note that this has the side effect of clearing a Social Security Number for a person if the only thing updated is the guarantor's address or other info.
    • Clicking this button will overwrite any existing patient-level guarantor information for that patient.
    • If you do not make any more changes to the order guarantor information, Harvest can update it with the latest patient guarantor information once the order is finally released into Harvest.
    • When you use this button, Harvest Webstation records the change in the Change Log in Harvest.
  4. Click the Save button to save your changes and return to the preceding page. Click the Cancel button to close the page and return to the preceding page without creating a guarantor record.
